What inspection reporting requires, why paper DVIRs fail audits, and how to design an electronic process that produces genuine safety assurance.

The driver vehicle inspection report is a simple document with disproportionate consequences. It is among the first things an auditor asks for, among the most commonly falsified records in transport, and among the easiest processes to fix.

What the record must show

Requirements vary by jurisdiction, but a defensible inspection record consistently includes:

  • The vehicle and, where relevant, the trailer
  • The driver who performed the inspection
  • Date and time, contemporaneous with the inspection
  • The items inspected
  • Defects found, described specifically
  • Whether any defect affects safe operation
  • Certification by the driver
  • Where a defect was found: the repair action, who performed it, when, and certification that the vehicle is safe
  • Review by the next driver where the vehicle changes hands

The chain from defect to repair to certification is what an auditor examines. A report showing a defect with no corresponding repair record is worse than no report at all.

Ask for the inspection records for one specific vehicle, on one specific date six months ago, together with the repair record for any defect found.

Why paper fails

  • Retrospective completion. A week of reports filled in on Friday, all in the same pen. Auditors recognise this immediately.
  • Illegible or generic entries. "All OK" for six months across a fleet of thirty vehicles.
  • Broken chains. The defect page and the repair record filed separately, and never reconciled.
  • Lost records. Cabs, weather, staff turnover.
  • No timestamp integrity. The date is whatever was written.
  • Retrieval cost. Producing eighteen months of records for one vehicle takes days.

Electronic inspection reporting addresses every one of these, which is why it has become the default in fleets of any size.

Designing the electronic process

Vehicle-specific check lists. A rigid, a tractor unit, a trailer and a van have different inspection items. Generic lists get ticked without reading.

Walkaround order. Follow the physical path around the vehicle. It improves both compliance and actual detection.

Mandatory photographs on high-risk items — tyres, load security, lights, coupling. Seconds to capture, transformative as evidence.

Defect severity at the point of reporting, with clear language: does this affect safe operation, yes or no. The driver flags; a competent person decides.

Immediate visibility. The defect appears in the workshop queue as it is raised, not at the end of the shift.

Linked rectification. The work order references the defect; closing the work order closes the defect with a certification.

Next-driver review. Where vehicles change hands, the incoming driver sees the previous report and any outstanding items.

Immutability. Records cannot be altered after certification; corrections create a new, attributed entry.

Trailer and coupled equipment

Trailers are frequently the weak point: inspected less consistently, moved between tractors, and often not linked to a specific driver for long. Make sure your process:

  • Records trailer inspections as separate, linked records
  • Captures the coupling — which trailer with which unit, and when
  • Handles the case where a trailer is dropped and collected by a different driver
  • Includes trailer-specific items: coupling, landing legs, load security, lights, brake lines

The behavioural problem

The hardest part is not the system. It is that a driver under time pressure, whose defect report will delay their departure and possibly their pay, has an incentive not to look too hard.

Counter it deliberately:

  • Never penalise a driver for finding a defect. Not in words, not in scheduling, not implicitly.
  • Close the loop. Tell the driver what happened to their report.
  • Monitor reporting rates by driver. Investigate the drivers who never find anything.
  • Compare driver-reported defects against those found at service. A large gap tells you checks are superficial.
  • Give the check enough time in the shift schedule. A check squeezed into a five-minute gap will not be genuine.
  • Recognise good reporting publicly, particularly when a report prevented a failure.

Records and retention

Retain for the period your jurisdiction requires — commonly a period of months to years — and consider longer where records could be relevant to a civil claim following an incident. Ensure records remain retrievable after a system change; an export you cannot read is not a record.

Common questions

Is a daily inspection required for every vehicle?

Requirements vary by vehicle type, weight and jurisdiction, and many regimes require a pre-trip check for commercial vehicles with a formal report where defects are found. Confirm the specific obligation for your fleet, and note that operating a defective vehicle is unlawful regardless of what the inspection regime requires.

What if no defects are found?

Most regimes still require a record of the inspection having been performed, even where nothing is found. A system that only creates records when defects exist cannot demonstrate that checks happened.

Who can certify a repair?

A person competent to assess the work, as defined by your jurisdiction and your own competence framework. Certification by someone without the relevant competence is a finding waiting to happen.

Can drivers use a personal phone for inspections?

Technically yes with a managed application, though it raises the questions covered in BYOD versus company devices. The evidential chain matters more than the device: attributed, timestamped, immutable records.

How do we handle defects found mid-journey?

Assess severity immediately. Safety-critical defects mean the vehicle stops where it is. Others may permit a return to base. The decision, who made it and on what basis must be recorded — this is exactly the sequence an investigator reconstructs after an incident.
